One measurement instead of three. Openings are not rectangles. Tile bows, wood racks, vinyl flexes. Measuring at three points finds the narrowest path the glass has to fit through, and the narrowest point is the only one that matters. Taking one measurement in the middle is how glass arrives a sixteenth too wide.

The wrong reference surface. Every job type measures to something different. A sash re-glaze measures the rabbet behind the stops. A sealed unit measures the visible glass and adds half an inch for what is hidden. A shower measures the finished tile after grout has cured. Measuring the visible glass when you needed the rabbet leaves you nearly an inch short every time.

Forgetting the deduction. Glass cut to the exact opening does not fit. It needs clearance for expansion and for the setting blocks that carry it. Glass expands and contracts with temperature, and a pane wedged tight into a frame develops edge stress and cracks — usually on the first genuinely hot afternoon after installation.

None of these matter much on annealed glass, where a fabricator can trim a sixteenth off. All of them matter enormously on tempered, which cannot be touched after it leaves the furnace. That is the whole reason we template rather than trust a drawing on anything with hardware in it.
